Hey, I’m trying to get my discharge status upgraded and have a few questions on completing the DD Form 149. Some of the sections are a bit confusing, like what to put in boxes 12, 13, and 18. Anyone gone through this before?
Yeah, I filled one out a while back. For box 12, definitely check ‘Discharge/Separation’ since you’re asking for a review of your discharge status.

Yep, box 12 is for the discharge review. It’s pretty common for this kind of request.
For box 13, just keep it simple. Something like, ‘Requesting discharge status be reviewed and upgraded to honorable due to changes in personal circumstances.’

I’ve seen people include a few sentences about how they’ve changed since then. But yeah, short and to the point works.
If you had any charges that impacted your discharge, you might have to mention them in box 14. It’s a tricky one—might be good to check with legal support if you’re not sure.

Smart move! Better to be safe with legal advice on that stuff.
For box 18, you could say something about why you think your discharge should be reconsidered, like personal improvements or recommendations from others. Attach anything you’ve got to back it up.

Definitely include any positive stuff you can. The board likes to see how you’ve grown since your discharge.
If you’ve already submitted your appeal, sections 4-6 might not need more info unless they ask. But double-check to make sure everything’s in there.

Also, make sure you’ve got all supporting docs attached. They might ask for proof if anything’s missing.
