Can I get traveler's insurance with pre-existing conditions?

My spouse is about to go on a two-week vacation to Hawaii, but they have some pre-existing medical conditions. I’m not sure what traveler’s insurance options are available in this case. Anyone know what I should be looking for?

It’s a letter from the doctor confirming that the condition isn’t an issue for travel. Basically just to prove it’s stable enough for the trip. It doesn’t need to be anything too complicated.
