Does a church need to complete wage forms for part-time workers?

I’m wondering about wage verification forms. A part-time employee was hit by a car on their day off and filed a claim with the driver’s insurance. Now, they need wage info, but the employee only worked as a substitute teacher at a church with no HR. Does the church have to fill out the forms in this case? Any advice? :confused:

Is the employee asking for wage compensation from the driver’s insurance?

Yep, that’s what it looks like. They might get something, but it’s not 100% certain.

In that case, the employee probably needs to show some kind of work history or pay stubs. Does the church have anything like that?

Can you calculate the total hours worked and divide them over the weeks before the accident?

Yeah, she worked like 2-4 hours a week, but it wasn’t consistent. I guess I could try that.

Has the church been ordered to give this info by the court?

Nope, they just got a letter in the mail asking for info. Nothing official like a court order.

In that case, the church isn’t obligated to do anything. They can provide as much or as little info as they want until a subpoena comes in.

Thanks! So, the employee can just send their paystub to the insurance?

@Lucypiper
Exactly. If that’s enough for the insurance, it should be fine.

Does the church have to complete that NYS NF-6 form for the employee?

I think it depends on whether they were officially asked to do it.

Yeah, unless there’s a formal request, they can just skip it for now.
