Manager Forged My Signature on My Resignation Form – Seeking Legal Advice

Hey everyone, I work at a Domino’s and I’m in a tough spot regarding my resignation. I texted my manager to submit my two weeks’ notice, and he filled out the official resignation form and signed my name without my permission. When I confronted him, he said I could fill out a new form when I came in, but the forged one would still be on file. I’m worried this could affect my employment record and future references.

Here’s what I’ve done so far:

  • Documented everything, including the text message.
  • Asked HR for a copy of the original form, but haven’t received it yet.
  • Requested that the forged form be removed from my file, but got no clear response.

I’m hesitant to fill out a new form until I know the forged one will be removed. I’m also considering legal advice if HR doesn’t address this properly. Has anyone dealt with something like this? He didn’t put anything untruthful on it, but it’s still frustrating. I plan to submit a new form stating that this is the only one I signed.
