Just go to ssa.gov and look for the ‘my Social Security’ portal. It’s pretty straightforward.
If you can’t find it online, you can also call them to request a replacement form. Just be prepared to wait a bit.

You can contact the Social Security Administration at 1-800-772-1213. They should help you out.
I had the same issue last year. Setting up my online account really helped, but it took a few days to get the hang of it.

Nah, it’s pretty user-friendly. Just make sure you have your SSN and other info ready.

Box 3 shows your total benefits paid, and Box 5 shows net benefits. Super important for taxes.
