The father of our child did not get involved at first and gave it a go later. He wants to leave now. Can he just give up child support and parental rights? I wish to be sure of that. Do I need to hire a lawyer?

Parental rights cannot be terminated using a court form. Either by yourself or with the help of a private attorney, you must draft the necessary pleading. In general, parental rights termination is covered under Family Code section 7820.
