What do I do if my FMLA forms got messed up?

So, my Dr’s intern totally botched my FMLA forms yesterday and sent in stuff saying I’m healthy and can work for 5-hour days. They even messed up my short-term disability forms after my recent hospitalization, only putting 3 days. My pain levels are really high right now, and I need time off for radiation therapy and PT for the next 1-2 months. Can my doctor fix and resend these forms? Will my employer have to accept the new ones?

Wow, that’s rough. Your doc should definitely fix that. Just explain the mix-up to them.

They should have! I mean, who messes that up? You deserve the right forms.

Yikes, that sounds stressful. Your employer really needs to accept the new forms, especially with a medical issue. Have you talked to HR?

Not yet. Just worried they might think I’m lying about my health now.

I get that fear. Just remember your medical stuff is private, so they can’t disclose that without your okay.

I’d say get your doctor to update the forms ASAP. Better to sort this out now than later. Trust me.

Good point! I just don’t know what they’ll say when they see the updates.

I think your doctor should send a note explaining the error too. Just in case, you know?

Yeah, that sounds smart. I just hope it doesn’t complicate things more.

Quitting isn’t the answer, especially if you need that insurance. Keep pushing for those forms to be fixed!

I’m trying, but it feels like a lot to handle right now.
